Elderly.jpeg The Needs Assessment & Service Co-ordination (NASC) is based at Greenlane Clinical Centre. The team works with elderly people with a disability and their family/whanau to determine their long term support needs and the needs of their carers. This service is for people over 65 years living in the Central Auckland area. The aim is to assist older people to remain living at home, safely and independently, for as long as possible.
A Needs Assessor will visit the client and work through an assessment. The assessment identifies tasks that the person finds difficult, and discovers what support others eg. family/whanau, community support groups etc, can provide. The assessor will then identify which needs will require support from formal services. Needs Assessors also refer for specialist assessments e.g. occupational therapy, physiotherapy to ensure that the person's own abilities are maximised. 
Areas of assessment include: health problems, personal care, domestic management, mobility, communication, support networks, recreational, spiritual, social situation and caregiver support.
Referrals are accepted for a person aged over 65 years of age who has been assessed as having a physical, neurological, intellectual, sensory disability or a combination of these. The disability is likely to continue for a minimum of six months and may result in a reduction of independence to the extent that ongoing support is required, and that will last longer than six months. 
Needs Assessors can come from a range of professional backgrounds, including Social Work, Occupational Therapy and Gerontological Nursing. Currently, the team has a majority of staff with a Social Work background. A mix of professional skills enables a high quality service to be delivered to clients.
